To qualify for a home care package, you must be:
Call My Aged Care on 1800 200 422 to start the process of getting a home care package.
A customer service representative will ask for basic information such as age, health, and living arrangements.

An Aged Care Assessment Team (ACAT) will contact you to arrange an assessment of care needs.
This assessment will determine the level of care required and whether a home care package is appropriate.
After a home care package is approved, you or your loved one will be placed in the national priority queue and notified when a package becomes available.
If not approved, other options for care will be discussed.

Clinical Assessor is a Home Care provider, and we will help you by developing a personalised care plan.
Your agreement with Clinical Assessor will set out the terms and conditions of the home care package.
Once the home care agreement is signed, visits from Clinical Assessor skilled care workers will commence.
Care plans are reviewed regularly.
The plan can be adjusted if necessary, and you can request a reassessment if your needs change.
